Normally I work out a general summary of what I mean to do, then start writing, and the details can be different from my anticipation. So there is considerable flow, but always within channels.
In my own work, when I start off writing a scene, I don't know which physical details are going to turn out to be meaningful. But, inevitably, certain images will stand out - you start to decide which ones are important as you go.
I won't start quite at the beginning... but the emigration of my parents from Nazi Germany and their new life in the U.S.A. A one sentence summary of these events is that after some years of trouble and considerable hard work, my parents established a satisfactory if not comfortable life for themselves and their two children.
If you have to find devices to coax yourself to stay focused on writing, perhaps you should not be writing what you're writing. And if this lack of motivation is a constant problem, perhaps writing is not your forte. I mean, what is the problem? If writing bores you, that is pretty fatal. If that is not the case, but you find that it is hard going and it just doesn't flow, well, what did you expect? It is work; art is work.
Writing has always been so much fun for me, and it still is. I think if you can keep it fun, then you have something. You start to lose it if it becomes work. That's one of the reasons that we're in this business - to get out of work.

The thing is that I don't normally think in terms of manga when I'm writing. Sounds odd from someone who has is getting a reputation for doing manga related work. But I would say that my scripts are NOT manga at the stage of my writing process, they are just comic book stories in a more general sense.
Everyone's [ me, Iain Cook and Martin Doherty] equally involved in all the writing. Normally we'll start with a sample or a drumbeat, or a synth sound or something like that, and that will spark the initial idea. I think either you're creative or you're not. In general, I don't think you need to be in pain to actually be creative unless you're writing love songs. Then you might need to have some ups and downs within your emotions to start to capture that.
The general view is that actors start on soaps and then maybe graduate to prime-time television or film; normally you don't see a film actor going to do a soap.
Then there is the further question of what is the relationship of thinking to reality. As careful attention shows, thought itself is in an actual process of movement. That is to say, one can feel a sense of flow in the stream of consciousness not dissimilar to the sense of flow in the movement of matter in general. May not thought itself thus be a part of reality as a whole? But then, what could it mean for one part of reality to 'know' another, and to what extent would this be possible?
I usually start writing a novel that I then abandon. When I say abandon, I don't think any writer ever abandons anything that they regard as even a half-good sentence. So you recycle. I mean, I can hang on to a sentence for several years and then put it into a book that's completely different from the one it started in.
People only watch six to eight to 10 channels, so if you want to be one of those channels, then you have to create content so strong that people have to come not once, not twice but enough that, behaviorally, they start to feel like, 'That's my channel.'
You don't start out writing good stuff. You start out writing crap and thinking it's good stuff, and then gradually you get better at it. That's why I say one of the most valuable traits is persistence.
[Writing something original from scratch], the initial process is way different. But once it exists and you start to actually work on making it real, then the approach is kind of the same, for me anyways.
